**PLEASE READ THE FOLLOWING INFORMATION CAREFULLY:

 To complete your Enrolment, each student must pay an Annual $40 Enrolment Fee before commencing classes. This fee reserves your individual place within selected classes and covers administration set up, invoicing, handouts/notices, concert information, first aid supplies and APRA/AMCOS (music rights).


All students and parents/guardians must read and agree to abide by all SRAD policies, procedures and rules of conduct as noted in the current year's Prospectus, as instructed and as listed below

 

Conditions of Enrolment, Permissions and Indemnity Waiver

I, the parent/guardian of the student named in the Enrolment agree to the following:

  1. Students are required to follow the schools uniform and conduct regulations, including being well Groomed and arriving on time for classes
  2. Due care will be taken with your child while they are in classes, however in case of illness or injury, where parents can’t be contacted in a medical emergency, I agree to any emergency medical treatment, ambulance or hospital services that may be required. All costs at parent’s/ Guardian expense.
  3. SRAD take no responsibility for loss or damage to personal property.
  4. Students & Teachers Dance Entirely At Their Own Risk.
  5. I agree to attend all rehearsals, except in the case of illness/injury (If you wish your son(s)/ daughter(s) to participate in the Annual concert on Date:vTBA  - Fee $40 per student payable in Term 4.
  6. Costumes remain the property of the school I understand that a non-refundable fee for costume hire applies and that payment can be made in full, as per the Information booklet. I understand that additional costume hire fees will be payable for competitions and is non-negotiable.
  7. I undertake to pay all fees as they are due for payment, for any period which the above student is enrolled at SRAD.
  8. I will read all SRAD Term Newsletters, SRAD Prospectus and email communication and respond Respectfully.
  9. I will treat all faculty, staff and members of SRAD with the utmost respect and value their experience and knowledge as teachers and trained professionals.
  10. I agree to maintain a positive atmosphere for all students and community members by refraining from making negative comments about the school, teaching faculty, processes, choreography, costuming etc. or about other students at any time, especially when present at the Studio.
  11. I will not dispute class placement and respect that all placements have been derived from many years of teaching experience and many factors have been considered when placement selection occurs.
  12. I agree my role as a ‘dance’ parent is to support my child. I also agree that my role as a ‘dance’ parent is to support our school, each other, our faculty and our Principal Director Shirley Rogers. I will acknowledge each decision is made with good intent and is in the best interests of my child.
  13. I will use and promote all SRAD social media accounts appropriately and respectfully.
  14. I will provide one term’s notice if I choose to leave SRAD.
  15. I agree for photos of my child to be taken and used on SRAD website. Photo/s in newspaper articles, Facebook, display boards. social media and/or any other promotional or advertising use for SRAD SRAD reserve the right to terminate any student’s enrolment at any time for misconduct or inappropriate actions by either the student or his or her parent/s.
  16. I have read the Ballet Exam information and would like my child to undertake their RAD Ballet Exams. I agree to support and respect the Principles final decisions on student’s ability and commitment on fulfilling the School’s RAD Ballet exam conditions of enrolment.
  17. I have read and clearly understood the general rules, policies and conditions regarding Shirley Rogers Academy of Dance.
  18. Students may not undertake dance training at another dance school in any dance style offered at the Academy while enrolled at SRAD.
